is closing effective August 31, 2023.
Frontier Dermatology Partners (FDP)/Silver Falls Dermatology will become the custodian of your medical records after August 31st. A transition of medical records takes several weeks, so they recommend that you wait until November 1st to reach out to them with any records requests. However, you are able to access your medical records at any time by using your Patient Portal. This is the quickest way for you to obtain your medical records, and you can access them through this portal without having to sign an additional release of information.

If you need to create a new login profile, you can contact the medical records division of Frontier Dermatology Partners at 866-599-3376 who can help set that up.
When records become available after November 1st, please note you must complete a Release of Information form through FDP; they cannot accept an old release from Dr. Fader’s practice. The Release of Information form can be found on their website:

FDP is a multi-office provider of medical and surgical dermatology services with many convenient locations: frontierdermpartners.com.
Other dermatology offices in the area include the Polyclinic in Ballard and Northgate, Overlake Hospital in Bellevue, and Providence Swedish in Bellevue and Issaquah.
FDP will not maintain or have access to any financial or billing records. For any questions pertaining to those matters, please contact West Coast Dermatology Billers Customer Service at 1-800-660-4884.

Samantha and Vanessa (Front Office Staff)
Samantha is the main front office medical assistant since 2003. Her duties include answering phone calls, resolving patient issues, medical records, and all front office administrative duties. Vanessa answers phone calls, resolves patient and staff issues, and is the patient liaison with our outsourced billing service.
Kyonna and Nicole (Nursing Staff)
Kyonna and Nicole perform all clinical medical assistant duties. This includes surgical assisting, coordinating care for patients, pre- and post-operative education, and all other nursing activities.
Loseli (Histotechnologist and Lab Manager)
Loseli is responsible for keeping the lab up to code, testing with CLIA (state agency), all lab inventories, notifying patients of biopsy results, some surgical scheduling. She was also the lead surgical medical assistant in the office from 2003-2013.
Amy (Administrative Officer)
Amy is the referral coordinator. Her responsibilities include insurance issues, authorizations, referrals, coordinating care, and other front/back office administrative duties. She often spends hours on hold with insurance companies – so you don’t have to.

About Insurance Plans
"We work with your insurance carrier to make the insurance process as smooth as possible."
Dr. Fader accepts most insurance plans except DSHS, Medicaid, and state-assisted plans. Please call our office or your insurance carrier to clarify.
Items to bring with you to your appointment:
- Insurance Cards — we need to make copies for our files
- Completed patient Health History form and Patient insurance forms
- Signed acknowledgement of receipt of the Privacy Notice
Co-payments and Deductibles:
- If your insurance requires a co-payment, please be prepared to take care of this when you check into the clinic. For the convenience of our patients, we accept Visa, Master Card, checks and cash. Should you need to make special financial arrangements, please contact our office.
Referrals:
- If you are a member of a managed care plan or HMO, a referral will need to be obtained from your primary care physician prior to seeing you in the office. Please check with our office prior to the day of your appointment to verify we have received the referral. If you arrive to see the physician and the referral has not been received, we might have to reschedule your appointment.
Please check with your insurance plan to confirm Dr. Fader participates with your insurance plan. Your plan may have you pay a larger portion of the charges if the doctor is not participating within your specific insurance network plan.

You are encouraged to print and fill out the Patient Health History Form, any applicable Insurance Forms, the Referral Information Sheet, and the Privacy Notice prior to your visit with us. This will save you check-in time at our office. In addition, if you have not been seen in our office for at least a year, we suggest completing these forms prior to your appointment so that we have updated information.
